CannAmm Occupational Health Services is committed to safeguarding the personal information provided by our clients. We manage personal information in accordance with the Personal Information and Electronic Documents Act and Health Information Act and other applicable laws. This policy applies to CannAmm Occupational Testing Services and to any person providing services on our behalf.
Accountability
Every person that provides services on CannAmm OTS' behalf is responsible and accountable to protect the privacy of personal information. A designated Privacy Officer is responsible for ensuring members of CannAmm Occupational Testing Services understand their responsibilities with regards to safeguarding private information. The Privacy Officer is available to answer questions about the application of privacy laws to individual situations as well as to receive concerns or questions about compliance.

CannAmm Occupational Testing Services collects, uses, and discloses information from clients for the purpose of managing our business. For example: In order to support the rehabilitation of employees providing test results to a client, their physicians, or other authorized individuals.
Consent
CannAmm Occupational Testing Services obtains an individual’s consent before collecting, using or disclosing personal information. The client’s consent is implied when presenting a requisition form for disclosing information or when providing services at the request of a healthcare practitioner. All other consent must be explicit.
Limiting Collection
CannAmm Occupational Testing Services only collects the personal information required to identify our clients in order to manage business and provide services.
Limiting Use, Disclosure and Retention
CannAmm Occupational Testing Services uses or discloses personal information only for purposes identified above, with the consent of the client, or as required by law. Personal information shall be retained only as long as necessary for the fulfillment of legal requirements or as required by our client’s needs.
Accuracy
CannAmm Occupational Testing Services collects and maintains personal information that is accurate, complete and as up-to-date as possible. This information is maintained and received from clients each time they use our services. Personal information is updated on each new service encounter.
Safeguards
CannAmm Occupational Testing Services ensures, through a variety of security safeguards, that personal information is protected against loss, unauthorized access and disclosure.

CannAmm Occupational Testing Services makes readily available specific information about its policies and practices related to the management of personal information
Individual Access
Upon request, CannAmm Occupational Testing Services shall inform clients whether or not it holds personal information about individuals except where permitted by law not to disclose personal information to such an individual.
Challenging Compliance
Clients may direct challenges and inquiries about our policies and practices concerning
compliance with the above principles to CannAmm Occupational Testing Services. All concerns will be investigated
and the results communicated to the appropriate parties.
